What information do I need to complete a leave request in Tilt?
Everything the leave survey asks for, section by section — gather it all before you open the form.
Quick answer
The leave survey is organized into sections — employee basics, type of leave, leave range, employment info, salary, work schedule, location, benefits, FMLA eligibility, and any additional files. Gather this info before you open the form.
Who this applies to / Prerequisites
- You are an HR user initiating a leave request on behalf of an employee.
Steps (info to gather, in the order the form asks)
- Employee basics
- Select an existing employee OR click Add New Employee to create a new profile.
- For a new employee: first name, last name, work email, phone, personal email, employee ID.
- Manager name.
- Type of leave
- Parental, medical, caregiving, military, or other.
- For parental leave: whether the employee is the one who is pregnant, type of birth, expected due date or date of adoption.
- For caregiving leave: which family member the employee will be caring for.
- Leave Plan Duration: continuous or intermittent.
- Leave Range
- Employee's Anticipated Leave Date (required).
- Expected return to work date (optional — can be filled in later by the employee).
- Note: if the leave is intermittent, the Leave Range should span the entire period from the first day off to the expected return, even if the employee will be working in between.
- Employment info
- Date of Hire.
- Job Title.
- Exemption Status: Exempt or Non Exempt.
- Pay Classification: Hourly or Salaried.
- Pay Schedule (dropdown, requires your company's Pay Periods to be entered in Tilt first).
- Job Classification: Full Time, Part Time, Temporary, or Seasonal.
- Current Salary
- Click MANAGE PAY to open the salary modal.
- Enter the salary Amount, Effective Date (required — this is the date the current salary took effect), and Period (year/month/week/hour).
- Additional information about the leave
- Free-form field for anything helpful to share about the leave (context, special circumstances, etc.).
- Work schedule
- What is the employee's typical work schedule?
- Set Schedule: the employee works the same schedule each week. Selecting this triggers follow-up fields for hours per day and hours per week.
- Unpredictable Schedule: the employee's schedule changes each week.
- Location
- Country.
- State or territory of residence.
- State or territory where the employee physically works.
- City where the employee physically works (optional).
- State-specific earnings: if the state of residence is Colorado, Connecticut, Massachusetts, New Jersey, Oregon, or Washington State, an additional field appears asking about the employee's earnings for the state's required base period.
- Benefits enrollment
- Company health benefits: Yes or No.
- Short-term disability benefits: Yes or No.
- Long-term disability benefits: Yes or No.
- FMLA eligibility
- Has the employee worked at least 1,250 hours in the last 12 months? Yes or No.
- Has the employee taken any FMLA leave in the past 12 months? Yes or No.
- Additional files (optional upload)
- Click UPLOAD FILES to attach pay stubs, medical documentation, or anything else that helps Tilt build the leave plan.
What if it doesn't work
- If you don't have some of the information, enter placeholders, and the employee will correct them when they complete their portion of the survey.
- If the Pay Schedule dropdown is empty, add your company's Pay Periods to Tilt first. See How do I add a New Pay Period Schedule?
- If the Current Salary section shows a red exclamation mark on the Effective Date, click MANAGE PAY and confirm you've entered a valid date.
- If an employee hasn't been with the company long enough to provide the number of pay stubs the state requires, upload what you have. The Leave Specialist will help fill the gaps with the employee.
Limits and exceptions
- Fields marked with a red asterisk (*) are required. Fields without an asterisk are optional.
- Some fields become required based on responses to other fields (for example, parental leave adds pregnancy and due-date fields; a state like California adds a state-specific earnings field).
- Job Classification is a multi-select — you can check more than one option if it applies (for example, Part Time AND Temporary).
- The employee will be able to review and update most fields when they log in to complete their portion.
- The Additional Information textarea and the Upload Files section are optional. You can leave them blank if there's nothing extra to share.
