If you request medical, disability, or caregiving leave that isn't covered by FMLA, your employer may require a medical certification from the relevant healthcare provider (yours, or your family member's). This applies if you're eligible for unpaid, job-protected state leave (like CFRA) or a company leave.

Steps
- Have the relevant healthcare provider complete the medical certification.
- Ensure it's signed and includes all necessary information, such as leave dates and the need for leave.
- Upload the completed certification to your Tilt leave plan.
What if it doesn't work

- If none of these apply, contact your Leave Success Manager.
